Health Benefits of Prostate Massage | What the Research Says

What does science say about prostate massage benefits? An honest look at the evidence, traditional Thai practice, and what to expect during a professional session.

Health Benefits of Prostate Massage | What the Research Says

Health Benefits of Prostate Massage

Prostate massage is one of the most asked-about yet least understood massage modalities. Men are curious about it for health reasons, pleasure, or both — but clear, honest information is hard to find.

This guide covers what medical research says, what traditional Thai medicine has practiced for centuries, and what to actually expect if you decide to try it.

What is the Prostate?

The prostate is a walnut-sized gland located below the bladder and in front of the rectum. It produces fluid that makes up part of semen. Every man has one, and it plays a role in both urinary function and sexual health.

The prostate is sensitive to touch and can be stimulated externally (through the perineum) or internally. This sensitivity is why prostate massage exists as both a medical procedure and a wellness practice.

What Medical Research Says

Let's be straightforward: the clinical evidence for prostate massage benefits is limited but growing. Here's what we know.

Supported by Research

Chronic prostatitis/pelvic pain: The strongest evidence supports prostate massage for chronic prostatitis (inflammation of the prostate) and chronic pelvic pain syndrome (CPPS). Studies have shown that massage can help drain fluids, reduce inflammation, and relieve discomfort in men with these conditions.

Improved blood flow: Massage increases circulation to the prostate and surrounding pelvic area. Better blood flow supports tissue health and may help reduce congestion.

Pelvic floor relaxation: Many men carry chronic tension in the pelvic floor without realizing it. Prostate massage can help release this tension, which may improve urinary symptoms and sexual function.

Promising but Inconclusive

Erectile function: Some studies suggest improved erectile function following prostate massage, possibly due to improved blood flow and reduced pelvic tension. More research is needed.

Post-ejaculatory pain: Limited evidence suggests prostate massage may help men who experience discomfort after ejaculation.

Complement to antibiotics: For bacterial prostatitis, some urologists have used prostate massage alongside antibiotic treatment to improve drainage and drug penetration. This is done in a clinical setting.

Not Supported

Cancer prevention: There is no evidence that prostate massage prevents prostate cancer. Don't believe any provider who claims otherwise.

Cure for BPH: Benign prostatic hyperplasia (enlarged prostate) is not treated by massage. Medical management or surgery is appropriate for BPH.

We believe in being honest about what massage can and cannot do. Anyone claiming prostate massage cures serious medical conditions is not being truthful with you.

Prostate Massage in Thai Traditional Medicine

While Western medicine is still studying prostate massage, Thai traditional medicine has incorporated it for centuries through a practice called Jabkasai (also spelled Kasai or Jap Kasai).

What is Jabkasai?

Jabkasai is a traditional Thai therapeutic massage focused on the male reproductive system. It involves massage of the abdomen, inner thighs, and pelvic area, including work on the inguinal ligaments and lower abdominal energy lines (sen lines).

Traditional practitioners use Jabkasai to address:

  • Low energy and fatigue
  • Reduced sexual vitality
  • Lower abdominal tension
  • Urinary difficulties
  • Impotence (in traditional understanding)

The Thai Perspective

Thai traditional medicine views the body through the lens of energy lines (sen). Blockages in the lower abdominal sen lines are believed to cause stagnation affecting the reproductive and urinary systems.

Jabkasai aims to restore energy flow through this region. While the theoretical framework differs from Western anatomy, the practical techniques overlap significantly with what modern physiotherapy considers pelvic floor work.

Cultural Context

Jabkasai has been practiced in Thailand for generations, traditionally by male healers working with male clients. It was considered a normal part of men's health maintenance, similar to how Thai massage addresses general musculoskeletal health.

Today, the practice continues in both traditional and modernized forms throughout Thailand.

Potential Benefits of Professional Prostate Massage

Based on available evidence and clinical observation, men who try professional prostate massage commonly report:

Physical Benefits

  • Pelvic tension release — reduction in chronic tightness through the pelvic floor
  • Improved circulation — better blood flow to the prostate and surrounding area
  • Reduced discomfort — for men with chronic pelvic pain or prostatitis symptoms
  • Sexual wellness — improved sensation, stronger response, and enhanced awareness
  • Urinary improvements — some men report easier urination and less urgency

Mental and Emotional Benefits

  • Stress reduction — the deep relaxation response that any quality massage provides
  • Body awareness — greater connection to a part of the body most men ignore
  • Anxiety relief — particularly for men who carry worry about prostate health
  • Curiosity satisfied — many men are simply curious and feel better after trying it

Who Should Not Get Prostate Massage

Prostate massage is not appropriate for everyone. Avoid it if you have:

  • Acute bacterial prostatitis — massage could spread the infection
  • Prostate cancer — consult your oncologist before any pelvic bodywork
  • Recent prostate surgery — wait until fully healed and cleared by your doctor
  • Hemorrhoids or anal fissures — the area needs to heal first
  • Blood clotting disorders — massage increases circulation, which can be contraindicated

If you have any active prostate condition, talk to your doctor before booking a prostate massage. We are massage therapists, not medical practitioners, and we'll always refer you to a doctor when appropriate.

What to Expect During a Session

Understanding the process helps reduce anxiety for first-timers.

Before Your Massage

  • Consultation — your therapist asks about your health history, experience level, and any concerns
  • Shower — you shower to cleanse and relax before the session
  • Communication — your therapist explains what will happen and answers questions

During Your Massage

A typical prostate massage session at Monkey Massage includes:

  1. Full-body warm-up — oil massage to relax your entire body, with focus on the lower back, abdomen, and inner thighs
  2. Abdominal work — massage of the lower abdomen following traditional Thai techniques
  3. Perineal massage — external stimulation of the perineum (the area between the scrotum and anus)
  4. Prostate work — gentle, gradual stimulation with constant communication about your comfort
  5. Integration — the session transitions back to full-body massage, allowing you to process the experience

Throughout the session, your therapist checks in with you. You control the pace and intensity. If anything feels wrong, you say so and the therapist adjusts immediately.

After Your Massage

  • Shower and take your time getting dressed
  • Drink water to support your body's processes
  • You may feel deeply relaxed, emotional, or energized — all normal responses
  • Mild soreness in the pelvic area is common for a day or two after your first session

Session Duration

DurationWhat It Covers
60 minutesFull-body warm-up with targeted prostate work. Good for experienced recipients.
90 minutesRecommended for first-timers. Allows proper warm-up, gradual pace, and integration.
120 minutesExtended session with thorough full-body massage and unhurried prostate work.

For your first prostate massage, 90 minutes provides the best experience. There's no rushing the warm-up phase.

Frequently Asked Questions

Is prostate massage painful?

No. When performed by a trained therapist at the right pace, prostate massage should not be painful. There is pressure, and the sensation is unfamiliar, but pain is not the goal. If you experience pain, tell your therapist and they'll adjust.

How often should I get prostate massage?

There's no fixed schedule. Some men come monthly as part of a wellness routine. Others try it once out of curiosity. For men managing chronic pelvic tension, more frequent sessions (every 2-4 weeks) may provide cumulative benefit.

Is this a medical treatment?

No. Prostate massage at Monkey Massage is a wellness service, not a medical treatment. If you have diagnosed prostate conditions, consult your doctor first. We complement medical care — we don't replace it.

I'm nervous about trying this. Is that normal?

Very normal. Most first-timers are nervous. Our therapists are experienced with first-time clients and move at your pace. You're in control throughout the session. There's no pressure to continue if you're uncomfortable.

Can I combine this with other massage types?

Yes. Prostate massage works well as part of a longer session that includes oil massage or Thai massage. The warm-up is essential to a good prostate massage experience, so combining it with full-body work is natural.

How is this different from a medical prostate exam?

Very different. A medical exam is clinical and brief — your doctor is checking for abnormalities. Professional prostate massage is a therapeutic, extended experience focused on relaxation, pelvic health, and wellness.
